Psychiatric disability evaluations

Psychiatric disability evaluations for established patients in Kansas.

If psychiatric symptoms affect work or daily functioning, an established patient can ask whether a clinical evaluation or documentation is appropriate. The request starts with the question or form involved, then a review of what clinical information is available.

House of Haven considers disability-related evaluation and documentation only for patients already established with the practice, based on clinical findings, available records, and professional judgment. New patients cannot book a standalone disability paperwork visit.

Established patients only. Evaluation, forms, reports, and benefit outcomes are not guaranteed.

Important note

Clinical documentation is not a determination.

For established patients only, House of Haven may consider independent clinical evaluation and documentation based on professional judgment. Starting ordinary care does not guarantee later forms. Disability eligibility, benefit decisions, VA ratings, insurance determinations, and legal outcomes are made by the applicable decision-maker. Information on this page is educational and is not legal advice.

Who this service may help

When mental health symptoms affect more than how you feel.

Established patients who are applying for benefits, appealing denials, or seeking functional-capacity documentation may discuss whether this service is appropriate. New patients can seek ordinary psychiatric care, without a future paperwork guarantee.

Possible clinical components

What an appropriate request may involve.

Clinical evaluation when appropriate

Review of relevant available records as needed

Mental-status examination

Functional-limitations assessment

Consideration of disability or RFC forms when clinically supported

Written clinical findings when appropriate

These are possible components, not a set package. The practice must confirm the scope, records, any forms, and any fees for each established patient.

Records that may be reviewed

Relevant records can help clarify the clinical picture.

The records needed depend on the purpose of the evaluation, the questions being asked, and what documentation is available.

Mental-status examination

The evaluation looks at more than a diagnosis.

A mental-status examination may assess current psychiatric presentation as part of a broader clinical assessment.

appearance and behaviorspeechmood and affectthought processthought contentattentionconcentrationmemoryinsightjudgmentorientationimpulse control where relevant

Functional limitations

Disability decisions often focus on function, not just diagnosis.

Psychiatric symptoms may affect daily and occupational functioning in ways that are not always obvious from a diagnosis alone.

The purpose of the evaluation is to document clinically supported functional limitations, if present, based on the available evidence.

RFC / disability forms

A form is considered only when clinical findings support it.

For an established patient, clinically supported disability-related or residual functional capacity documentation may be considered. No particular form or result is guaranteed.

Documentation must reflect:

Written clinical report

A written clinical report may be considered.

If a report is clinically appropriate and within scope, it would summarize supported findings and relevant available evidence. A report is not guaranteed for every request.

For attorneys

Representing an established House of Haven patient?

With the patient's authorization, an attorney may contact the practice about relevant records or clinically appropriate documentation. House of Haven does not accept a new client solely for a disability evaluation or form.

For veterans

Mental health conditions can affect work, relationships, concentration, sleep, and daily functioning.

Veterans already established with House of Haven may ask whether clinical documentation is appropriate. New patients cannot book a standalone VA or disability paperwork visit. VA benefit determinations and ratings are made by the Department of Veterans Affairs.

How it works

How to ask about evaluation or documentation.

Be an established patient.
New patients cannot schedule a standalone disability paperwork evaluation.

Contact the practice.
Explain the purpose of the request and identify any specific form, deadline, or question. The practice can discuss whether the request is within scope.

Discuss available records.
If records are needed, ask which are relevant and how to provide them securely. Do not send clinical details through a general website form.

Clinical review if appropriate.
Niesha Taylor, MSN, APRN, PMHNP-BC, determines whether an evaluation is appropriate and what clinical history, examination, and functional concerns to assess.

Documentation decision.
Any written findings or form depend on the available evidence, clinical judgment, and professional scope. The practice does not decide benefits eligibility.

FAQ

Questions about psychiatric disability evaluations.

These answers are educational. Decision-makers such as agencies, insurers, employers, and courts make disability or benefit determinations.

What is a psychiatric disability evaluation?

For an established House of Haven patient, a request begins with discussing the purpose and any specific questions or forms. If an evaluation is appropriate, it may consider symptoms, history, relevant available records, and day-to-day functioning. The scope and any documentation are determined individually.

What is an RFC?

Residual Functional Capacity documentation describes what an individual may or may not be able to do consistently despite a condition. For an SSA claim, SSA assesses RFC from the evidence; a clinician may provide supported observations or an opinion about work-related functioning. A House of Haven RFC form is not automatic.

Does a diagnosis automatically mean someone is disabled?

No. Disability determinations generally involve functional impact as well as diagnosis. A psychiatric evaluation documents clinically supported symptoms and limitations but does not itself guarantee a disability determination.

Can House of Haven review my medical records?

For an established patient, relevant records may be reviewed when clinically appropriate. The scope depends on the case and available documentation.

Can House of Haven complete disability or RFC forms?

Only for already-established House of Haven patients, and only when clinically appropriate and supported by the evaluation, relevant disability or RFC documentation may be considered within professional scope. Completion is not guaranteed.

Can my attorney send records?

For an established patient, records from an attorney may be considered with appropriate authorization and a secure process. House of Haven does not accept new patients solely for disability paperwork.

Do you guarantee disability approval?

No. House of Haven provides independent clinical evaluation and documentation. Disability decisions are made by the applicable agency, insurer, employer, or other decision-maker.

Do you perform evaluations for veterans?

Established House of Haven patients who are veterans may ask whether clinically appropriate documentation is within scope. New patients cannot book a standalone VA or disability paperwork visit. VA benefit decisions and ratings are made by the Department of Veterans Affairs.

Can a new patient book a disability or RFC paperwork appointment?

No. These evaluation and documentation services are available only to patients already established with House of Haven. New patients may seek ordinary psychiatric care, but establishing care does not guarantee later forms, certification, or benefit approval.
