Work + mental health
FMLA-related evaluation and documentation for established House of Haven patients.
When anxiety, depression, stress, panic, sleep problems, or other mental health symptoms begin interfering with your ability to function at work, a psychiatric evaluation can help clarify what is happening and what support may be appropriate.
For patients already established with House of Haven, Niesha Taylor, MSN, APRN, PMHNP-BC may consider FMLA-related evaluation and documentation when clinically warranted. New patients cannot book a standalone paperwork appointment.
Established patients only. Clinical review first. No guaranteed certification or leave.
Important note
Evaluation is not a guarantee.
House of Haven considers FMLA documentation only for established patients, based on professional judgment. Starting ordinary care does not guarantee future paperwork. Clinical certification can describe supported medical facts and work limitations; the employer or leave administrator decides employee eligibility and whether leave is designated under FMLA. When work becomes hard to manage
When your mental health starts affecting your work, it can show up in many ways.
You may still be showing up, answering messages, and trying to keep everything moving while your symptoms are making work feel harder to sustain.
Conditions that may affect work
Mental health symptoms can affect focus, attendance, sleep, and daily functioning.
Anxiety
Depression
Panic symptoms
ADHD-related functional impairment
Trauma-related symptoms
Stress-related mental health symptoms
Sleep-related psychiatric concerns
Mood instability
Whether a condition supports FMLA-related leave depends on the individual's clinical circumstances, functional limitations, applicable eligibility requirements, and the documentation requested by the employer.
What the evaluation looks at
The goal is to understand how your symptoms are affecting your ability to function.
A psychiatric evaluation does not begin with a predetermined certification outcome. It begins with understanding symptoms, functioning, treatment needs, and what is happening in your life and work.
FMLA documentation
When clinically appropriate, medical documentation may be part of your care.
For an already-established patient, Niesha may consider appropriate clinical certification if the evaluation and available information support it. A new appointment or request does not guarantee documentation.
Employers may request documentation addressing areas such as:
Types of leave
Intermittent, continuous, and reduced-schedule leave each mean something different.
Continuous leave
A period away from work when symptoms or treatment needs make working medically inappropriate.
Intermittent leave
Time away from work in smaller increments when symptoms flare or treatment is required, if medically necessary and otherwise permitted under FMLA.
Reduced schedule
A temporary reduction in work hours when clinically necessary.
The type and duration of leave, if any, should be based on medical necessity and the employer's FMLA process.

Why patients may seek help
You do not have to wait until you completely fall apart.
Many people try to push through worsening anxiety, depression, stress, or other symptoms because they are afraid of missing work or appearing unreliable.
When mental health interferes with functioning, ordinary psychiatric care can help clarify what is happening and what treatment may fit. For an already-established patient, documentation can be discussed separately when clinically appropriate.
Start with clarity.

How it works
How to request a clinical documentation review.
Check the employer's request.
Ask your employer or leave administrator whether medical certification is required and which form or instructions apply. They also determine your employee eligibility.
Call if you are already a patient.
Contact House of Haven at 913-430-8911 about the work-related concern and requested documentation. Do not send clinical details through this website.
Discuss a clinical review.
Niesha reviews symptoms, functioning, treatment, and available information within her clinical scope. She decides whether any certification is supportable; a request or visit does not guarantee paperwork.
Follow the leave administrator's instructions.
Provide any completed certification through the requested process. Your employer or leave administrator, not House of Haven, decides whether the leave is designated as FMLA.
New to House of Haven? Review ordinary care scheduling. Adults can use Headway; parents or legal guardians scheduling for ages 10–17 are preferred to call the office. In-person care is at the Lenexa office; telehealth is for eligible Kansas residents physically in Kansas at visit time. Missouri residents may ask about eligible in-person Lenexa care. Ordinary care does not promise later FMLA documentation.
FAQ
Questions about mental health and FMLA.
These answers are educational and based on federal FMLA guidance. Your employer or leave administrator determines eligibility and designation.
Can anxiety qualify for FMLA?
Potentially. Under federal FMLA rules, qualifying mental health conditions may meet the definition of a serious health condition when applicable requirements are met. Individual circumstances and employer eligibility rules matter.
Can depression qualify for FMLA?
Potentially, depending on the severity, treatment requirements, functional impact, and other applicable FMLA requirements.
Can a nurse practitioner complete FMLA certification?
Under federal FMLA guidance, nurse practitioners may qualify as health care providers when authorized to practice within their scope.
Can I get intermittent FMLA for mental health?
Intermittent or reduced-schedule leave may be appropriate when medically necessary and when FMLA requirements are otherwise met.
Will House of Haven guarantee that my employer approves FMLA?
No. FMLA documentation is considered only for established House of Haven patients when clinically appropriate. Employer eligibility and FMLA designation decisions are handled through the employer or leave administrator.
Do I need an official diagnosis?
Employers may require sufficient medical certification, but a diagnosis is not necessarily required to appear on the certification. The clinical evaluation determines what information is appropriate to document.
Can House of Haven complete my employer's form?
For established House of Haven patients only, clinically appropriate forms may be reviewed and completed within scope based on the evaluation and available clinical information. Completion is not guaranteed.
Can I come only for paperwork?
No. FMLA evaluation and documentation services are for already-established House of Haven patients only. New patients may seek ordinary psychiatric care, but establishing care does not guarantee later certification, paperwork, or leave approval.
Can a new patient book a visit for FMLA paperwork?
No. This documentation service is limited to patients already established with House of Haven. A new patient can ask about ordinary psychiatric care; future paperwork depends on the clinical relationship and professional judgment and is never guaranteed.
How do I request a mental health FMLA evaluation?
If you are already a House of Haven patient, ask your employer or leave administrator what certification is required, then call the practice about a clinical review. Niesha decides whether the available clinical information supports documentation; your employer or leave administrator decides FMLA eligibility and designation.
